| Scheduling Your Appointment It's fun and easy to have a private one on one consultation from the comfort of your own home, car or office. To set up a telephone appointment, send an email to lifemattersNow@yahoo.com and include the time and day you would like to schedule your telephone appointment. Appointments can be scheduled between 12 noon and 12 midnight EST Monday through Friday. Special arrangements can be made for weekend or holiday appointments. An email confirmation will be sent to you confirming the time and day of your appointment. You can also call 330-781-0030 to schedule an appointment directly. Please read over Fees and Payment Policy, Limitation of Liability and submit Service Agreement below, before scheduling your appointment. Fees and Payment Policy Fees Consultations fees are based on $55 for an hour session. Sessions lasting longer than an hour will be billed in 15 minute increments of $13.75 each. Payment Payment is required prior to or at the time of services, depending on payment method. Payment may be made by check, cash, money order or credit card. There will be a $25 charge for any check with non-sufficient funds. If not paying online by credit card, a check or money order must be received prior to the scheduled session. Service Agreement PLEASE READ THE FOLLOWING AGREEMENT BELOW
Limitation of Liability The client agrees that use of any and all services provided by Life Matters and Laura Tellman is entirely at the client's own risk. Services are provided "as is", without warranty of any kind, either expressed or implied. Specifically, any and all warranties are disclaimed, including without limitation any warranties concerning the availability, quality, accuracy or content of information, products or services or outcome. This disclaimer of liability applies to any damages or injury caused by the failure of performance, error, omission, interruption, deletion, defect, delay in operation of transmission, computer virus or breakdown, communication line failure, theft, destruction or unauthorized access to, alteration of or use of record, whether for breach of contract, tortuous behavior, negligence or under any other cause of action. Neither the provider nor any of its agents shall be liable for any direct, indirect, special or consequential damages arising out of the use of the service or inability to gain access to or use the service, or out of any breach of any warranty. The client hereby acknowledges that the provisions of this section shall apply to all content on these services.
